Funders

As a visionary funder, your investment plays a critical role in transforming healthcare and enhancing community well-being. By supporting innovative projects that address the social determinants of health (SDOH), you enable healthcare providers, insurance companies, and community organizations to deliver impactful services with unprecedented precision. Your funding empowers these groups to leverage data analytics and technology, facilitating the development of healthcare models and community programs that are both effective and efficient.

Your contributions also help establish vital partnerships among government agencies, for-profit entities, and philanthropic organizations, greatly expanding the reach and effectiveness of these initiatives. Strategic collaborations ensure that every dollar you invest translates into meaningful, lasting benefits for underserved populations, creating a legacy of health, equity, and opportunity that makes a difference.

Organizations

You represent a transformative force in healthcare and community change, whether you're a healthcare provider, business leader, health-focused nonprofit, or advocacy group. You are deeply committed to enhancing health outcomes and cost efficiency. By developing human-centered healthcare models and leveraging data and technology that support the social determinants of health (SDOH) you can target interventions that address these determinants with new precision and care.

Embedded within your community, your organization—with its established credibility and strong local ties—is perfectly positioned to make a significant impact on health-challenged populations. By forging partnerships with government agencies, for-profit entities, and philanthropic organizations, you can expand your reach, turning your vision for a healthier community into a reality. This is your chance to lead in creating critical, sustainable changes in healthcare.

Where innovation meets compassion,

lasting health begins.

Services

  • Creating a new intervention is a patient process. We can increase the likelihood of a new SDoH program’s success and positive impact on the community. Careful planning, collaboration with stakeholders, adherence to legal requirements, and a commitment to continuous evaluation and improvement are essential. Key steps include:

Identifying or Clarifying the Need: Only when clearly defined, problem solving can begin. The need must remain the project’s north star.

Setting Objectives: Once the need is identified, specific objectives for the social program should be established. These objectives should be SMART (Specific, Measurable, Achievable, Relevant, Time-bound) to provide clarity and direction.

Designing the Program: This phase involves designing the structure and components of the program. Key considerations include (not necessarily sequential):

Target Population, Interventions, Process & Delivery Mechanisms, Budget & Funding, Legal and Regulatory Issues, Early pilot implementation, Implementation & Monitoring, Impact Assessment, Sustainability & Scalability.

  • Optimizing an existing intervention is also a patient process. Well Informed Consulting can assist with:

    - Strengthening Organizational Capacity

    - Growing leaders for expansion and succession

    - Building Partnerships across sectors

    - Tailoring programs to healthcare settings

    - Expanding existing offerings to embrace culturally relevant practices

  • By focusing on leadership growth, leadership transitions and succession planning, individuals and organizations can cultivate a robust leadership pipeline, adapt to changing circumstances, and sustain long-term success. Well Informed has a strong track record in emphasizing Leadership Culture for individuals and organizations.

  • Social Determinants of Health (SDoH) and Drivers of Health Equity:

Learn how over 80% of our health is driven by non-medical factors. This includes systemic issues such as racism, discrimination, and policies, highlighting the importance of addressing power dynamics and systems of oppression that underlie social determinants.

    Adverse Childhood Experiences (ACEs):

Learn how early life experiences can change the course of a person’s mental and physical health and wellbeing. Adverse Childhood Experiences affect mental and emotional health and studies show that stress and trauma can also manifest in physical ways – anything from chronic diseases to a shorter lifespan. The largest study to date that made this connection is the original ACE study conducted from 1995-1997 at Kaiser Permanente, a large health care company. The study found that childhood abuse, neglect, and household challenges can significantly impact later-life health and wellness.

    Early Childhood Impact:

Learn how from birth to age 5, a child’s brain develops more, and more rapidly, than at any other time in life. And while genetics plays a significant role, scientific research has made clear that the quality of a child’s experiences in the first few years of life – positive or negative – helps shape how their brain develops. And that these experiences have lasting impact on their health and ability to learn and succeed in school and in life.

    Individual & Family Wellness:

Thriving with Chronic Disease: In 2016 Dr. “Donna” Berkey Lowry was diagnosed with severe rheumatoid arthritis. Before symptoms were controlled she would have to crawl from the car to the couch at the end of the day. A few months into medication and diet changes, her son was diagnosed with brain cancer. Patiently managing one’s own chronic condition and a family member’s, too, can catalyze change that will support long term thriving. She’d love to share her ongoing story and facilitate the conversation many of us need.

Positive family experiences: In partnership with Pathwaves Partnership, we can offer an array of practical trainings and support to build positive experiences into everyday moments.

    The Power of Mentorship:

Learn the data on the power of mentorship across personal and professional sectors. Discuss your organization’s needs and explore mentoring best practices.

    Non-Profit Leadership:

With commitment and intention, leadership growth is within reach. Key areas of development include Organizational Communication, Negotiation, Mediation, and FacilitatioN, Leadership Development, Ethical Leadership, Strategy Development and Implementation, Organizational Transformation, Leading Teams and Change Management
